Job Description

The Primary Care Development (PCD) team supports MOH and primary care providers in the development and implementation of primary care models aimed at strengthening the sector to provide better care for patients and achieving the Primary Care vision of “One Singaporean, One Family Doctor”.

PCD is the appointed programme administrators for Primary Care models such as Community Health Centre (CHC) and Primary Care Network (PCN).

You will be managing key partners in the Primary Care Network (PCN) programme to:

  1. Strengthen team-based care within each PCN – Joint review and planning of practice model, service provision, operational workflows and fund utilisation.
  2. Strengthen administration of the programme in key areas such as programme management, claims management, governance, data reporting and analysis, etc.
  3. Co-develop new initiatives to support care integration

Key functions of the role are:

  1. Key account management
  2. Programme management and evaluation  
  3. Project management for new initiatives

Job Requirements

  • Degree holder in any discipline
  • Minimum 8 years of relevant working experience
  • Possess strong organisational skills in communication, time management, analytics and problem-solving
