Job Description

Broad Function

You will be part of a dynamic team to drive, plan and implement strategies and programs that will build the capability and capacity of healthcare professionals, key partners and caregivers to provide high quality community mental health care, in alignment of the missions of AIC.

You must work in partnership with a diverse group of stakeholders and partners including community and government agencies, social service agencies (SSA), healthcare institutions and the Intermediate and Long Term Care (ILTC) providers to identity the learning needs, resource requirements and capacity gaps to respond to the challenge of improving the lives of mental health patients and their caregivers.

Key Responsibilities and Desired Outcomes

1) Planning and development

  • Support the division in service planning and continuum of care for the integrated community mental health and dementia support network across the care sectors including social and healthcare sectors, particularly in the area of dementia

2) Scanning, research and key stakeholder engagement to identify trends and needs

  • Identify, conceptualise and propose new services and initiatives that complement the Community Mental Health Division’s work-plan and directives, particularly in the area of dementia.
  • Engage stakeholders to identify opportunities for collaboration that is in line with the Community Mental Health Division’s work-plan.

3) Programme management for pilots including programme review for sustainability

  • Drive the implementation and review of relevant community mental health pilots to achieve better health outcomes and improve the quality of lives of patients and caregivers.
  • Review and propose enhancements to relevant Capability and Capacity Building activities as prescribed under the Community Mental Health Division’s work-plan and directives
  • Provide funding management support for funding utilisation and ensure efficient use of funding by the community partners across the continuum including healthcare, education, social, security and other community agencies

4) Other AIC/Division work

  • Support the department/division in all related activities to meet the divisional objectives of CMH.

Job Requirements

  • Recognised Bachelor degree in healthcare-related discipline
  • At least 2 years’ experience in healthcare or community care sectors, preferred.
  • Prior experience in dementia or geriatric areas will be an advantage.
  • Good interpersonal and communication skills; proactive, responsible, good, team player and can work independently.
  • Good project management skills
  • Good writing and presentation skills
  • Competent in MS Office Word, Excel, and Powerpoint.
