Job Description

Responsibilities:

 Development of New Models of Care 

  • Recommend and plan new sector development to better meet the needs of primary care in the community
  • Drive the development of new models for primary care through collaboration with like-minded stakeholders to develop project proposals and implementation plans
  • Implement publicity plans and initiatives through various platforms to encourage adoption and utilisation of new models of care
  • Manage the programme evaluation efforts by coordinating, collating and tracking the outcome measures and performance indicators on a regular basis
  • Provide support in the management of funding for implemented projects and initiatives
  • Plan and implement standards for new models of care

Engagement of Stakeholders

  • Support the engagement of stakeholders such as Board members and external stakeholders
  • Assist in developing and implementing data collection systems that includes database of chronic conditions
  • Manage approved budgets for the programmes including timely submission of claims and delivery of reports from Primary Care Network (PCN) partners and relevant stakeholders
  • Manage the programme evaluation efforts by coordinating, collating and tracking the outcome measures and performance indicators on a regular basis
  • Provide relevant information to PCN partners and relevant stakeholders, and assist in addressing their queries and concerns
  • Seek feedback from stakeholders through regular communications to support process improvement initiatives

Job Requirements

Requirements: 

  • Degree in any discipline
  • Minimum 10 years of relevant working experience with at least 5 years in a managerial capacity
  • Prior experience in healthcare, project management or operations in primary care or hospital is desirable
  • Possess a high level of adaptability and learning agility in a dynamic environment
  • Experienced in working with diverse groups of people in an organisation
  • Possess strong organizational skills in communication, time management, analytics and problem-solving
