Job Description

Project manage and lead a team to design, develop, implement and evaluate Community Case Management Service (CCMS), support Care Coordination and Case Management initiative.

1.  Community Case Management Service (CCMS)

  • Review of existing models of case management in the community.
  • Work with internal and external stakeholders to evaluate and review needs and project demand for the services island-wide.
  • Project management for the development of CCMS providers.
  • Engage providers to implement the project.
  • Review project regularly to identify and understand gaps and issues in existing service.
  • Support evaluation of existing and future models of CCMS.
  • Monitor and track budget and KPIs
  • Oversee operations including referral management

2.  Support Care Coordination and Case Management (CCCM) Initiative

  • Cross divisional work to support projects (ServiceLinks, ComCAT and CCMS) under CCCM initiative in reports to MOH and Toteboard.
  • Work with internal stakeholders to prepare financial statements and status reports for the initiative. 

3.  Any other duties as assigned. 

Job Requirements


  • Degree in any discipline
  • Minimum 5 years of relevant working experience
  • Prior experience in healthcare, project management or operations in primary care or hospital is desirable
  • Possess a high level of adaptability and learning agility in a dynamic environment
