Job Description

Contribute to the development and delivery of home- and community-based services that support and enable clients to age in place.

1. Programme Management

  • Work with service providers to drive improvement in operational processes and delivery of care
  • Provide support in planning and implementation of stakeholder engagement initiatives
  • Manage project deliverables and work collaboratively with fund administrators
  • Assist in other responsibilities arising from management of programmes 

2. Stakeholders Engagement

  • Establish close communications and strong rapport with service providers and other relevant stakeholders
  • Collaborate with both internal and external stakeholders to implement plans and activities
  • Support the implementation of competency and standards for delivery of care
  • Seek feedback from stakeholders through regular communications to support improvement initiatives

3. Programme and Service Development

  • Collaborate with stakeholders to prepare and implement home- and community-based services
  • Track progress and ensure compliance to development and implementation timelines
  • Assist to source for funding from government and non-governmental agencies, including working with providers to craft proposals (if necessary)
  • Assist in other responsibilities arising from the development of programmes and services

Job Requirements


  • Degree in any discipline
  • Minimum 5 years of relevant working experience, candidates with experience in healthcare, project management or business development are preferred

  • Competency in basic statistics and MS Office applications

  • Possess strong interpersonal skills & ability to work with diverse groups of people

  • Knowledge in geriatrics and community services

  • Ability to work independently to help drive projects, with demonstrated adaptability and learning agility in a dynamic environment

  • Strong organisational skills in communication, time management, analytics, critical thinking and problem-solving are necessary

  • Ability to use local language and dialects would be an advantage
