Job Description

  • Provide leadership and manage Clinical Quality Improvement (CQI) projects to improve the quality of care delivered by Service Providers in the Community Care sector.
  • Critically review research articles; communicate and apply significant findings to the planning of the CQI project.
  • Drive and facilitate Quality Improvement (QI) projects, initiatives and workshops aimed at organisational improvement for community care providers and within AIC.
  • Facilitate and guide CQI project champions in the development, planning, implementation and review of care bundle/ care path/ training plans - on-site at Nursing Homes.
  • Apply and customize QI methodologies to suit the needs of the providers and to achieve the desired objectives.
  • Assist in the review and development of clinical and project-based audit tools.
  • Influence the sector's directors of nursing and clinical quality leads to drive improvement within their organisations and to come on board cross-sector programme.  Serve as a resource person in the assessment, planning, implementation and evaluation of CQI project-related patient care matters.
  • Collaborate with AIC QI team in facilitating CQI.
  • Work with community care partners to develop nursing protocols and provide guidance on the implementation of good clinical practices. 
  • Initiate and manage sector-wide events and platforms to build and sustain the quality improvement culture in the sector.
  • Promote successful improvements through the creation and/or delivery of presentations, articles, conference posters, etc.

Job Requirements

Job Requirements:

  • Degree in Nursing or a post-basic in Nursing-Education or Clinical sub-specialty.
  • At least 8 years' experience of clinical care leadership in a ward-based setting or in a clinical quality and safety role.
  • Relevant clinical experience in healthcare sector or community setting working with elderly population would be an advantage.
  • Significant experience in leading or facilitating QI projects.
  • Good interpersonal, communication and writing skills.
  • Good project management and analytical skills.
